Health Condition Management

Omaha Public Schools serves many students with chronic health conditions. Below are action plans and forms used to meet the health needs of students. A completed plan is required for medication administration in the school setting. It is the responsibility of the parent/guardian to provide updated action plans and/or medication orders annually. These plans must be completed by a health care provider and signed by the parent/guardian. Please work with your healthcare provider and school nurse in developing a specific plan to keep your child safe, healthy, and ready to learn.
