Immunization Requirements

The Omaha Public Schools follows the immunization requirements outlined by the Nebraska Department of Health and Human Services. A copy of each student’s immunization record must be presented to the school’s health office prior to attending school. Students who do not meet the state rules and regulations must have a medical waiver or a notarized religious waiver on file. Any student who does not comply with the state immunization requirements shall not be permitted to continue in school until proof of progress towards compliance is received. Below is a list of the required immunizations.
- Required Immunizations for Early Childhood Programs: (ages 2-5)
- 4 doses of DTaP, DTP, DT, or TD vaccine
- 3 doses of Polio vaccine
- 3 doses of Hib vaccine or 1 dose of Hib given at on or after 15 months of age
- 3 doses of pediatric Hepatitis B vaccine
- 1 dose of MMR or MMRV vaccine, given on or after 12 months of age
- 4 doses of pneumococcal, or 1 dose of pneumococcal given on/after 15 months of age
- 1 dose of varicella (chickenpox) or MMRV given on or after 12 months of age. If your child has had varicella (chickenpox), they do not need any varicella vaccines. Written documentation must be provided (including month and year) of varicella disease from the parent/guardian or health care provider
- Required Immunizations for Kindergarten-6th grade:
- 3 doses of DTaP, DTP, DT, or TD vaccine, one given on or after the 4th birthday
- 3 doses of Polio vaccine
- 3 doses of pediatric Hepatitis B vaccine
- 2 doses of MMR or MMRV vaccine, given on or after 12 months of age, separated by at least one month
- 2 doses of varicella (chickenpox) or MMRV given on or after 12 months of age. If your child has had varicella (chickenpox), they do not need any varicella vaccines. Written documentation must be provided (including month and year) of varicella disease from the parent/guardian or health care provider
- Required Immunizations for 7th -12th grade:
- 3 doses of DTaP, DTP, DT, or TD vaccine, one given on or after the 4th birthday
- 3 doses of Polio vaccine
- 3 doses of pediatric Hepatitis B vaccine, or 2 doses of adolescent vaccine if student is 11-15 years of age
- 2 doses of MMR or MMRV vaccine, given on or after 12 months of age, separated by at least one month
- 2 doses of varicella (chickenpox) or MMRV given on or after 12 months of age. If your child has had varicella (chickenpox), they do not need any varicella vaccines. Written documentation must be provided (including month and year) of varicella disease from the parent/guardian or health care provider.
- 1 dose of Tdap
