Students who wish to participate in school sports and athletics are required to have a sports physical each year they want to participate.

Requirements for sports physical:

• Seventh grade
  • Completed on or after March 1 for following year
  • Required school entrance physical can be used as long as it is completed on or after March 1
• Eighth through 12th grade:
  • Required to have sports physical each year they want to participate
  • Completed on or after May 1 for the following year

Signing a waiver does not satisfy this requirement. In addition to your child’s physician, many health care organizations offer physical clinics during the summer months.

Proof of insurance, either through the K & K Insurance company, or through personal/private health insurance coverage must be provided before a student is allowed to participate in Omaha Public Schools athletics programming.

To be eligible to participate on the first day of practice and events, please complete each form in the appropriate athletics packet and return them to your student’s school athletics department. Families can also complete the packet electronically in Infinite Campus.
